Think of it like two separate tanks, one filled with compressed air on the bottom and one with fresh brake fluid on top. You need to fill the bleeder first with fresh fluid, then add compressed air to the bottom of the diaphragm. When the valve is opened on the bleeder the compressed air at the bottom of the bladder pushes the fresh fluid located on top of the bladder up and out of the hose, into the master cylinder.
The fresh brake fluid enters the master cylinder under pressure and is pushed through ALL the hydraulic components of the brake system. Each bleed screw is opened and the entire system can be flushed of old brake fluid. The old fluid is pushed out of the system with fresh fluid behind it, this results in the complete replacement of all of the brake fluid in the system.
Bottom line? A pressurized brake bleeder not only removes air from the brake system, it flushes and removes the moisture and oxidation from the system. There really aren’t many drawbacks to talk about with this product, I have used one for years daily at the shop and it’s held up very well. Over time things that start to fail are the main bladder (which can easily be replaced) and the main valve, it can start to seep brake fluid over time.
